Application Instructions
To create a new account
- Click the New User button on the home page
- Enter your name
- Enter your email address
- Enter a password
- Click submit
Once you create your account, login and begin the application process.
Note: Your email address as our primary means of communicating with
you. Please make sure your email filters allow you to receive email
from support@luxapply.com.
Begin the application process
You can complete the application process in multiple sessions and in
any order you choose. Keep in mind that you can make edits to your
application at any time up until the application deadline.
- Select the section you want to work on from the list on the left-hand side of the page.
- Follow the specific instructions for each section.
- Proceed
to the next section by clicking "Save" at the bottom of the page. To
select a specific section, click "Save" and then click the option you
want from the list on the left-hand side of the page.
- End your session by clicking "Save" and then select "Log-out" from the left-hand side of the page.
- Login to return to your account. The application will appear as you left it during your last session.
- Edit, remove, or add new information to any section at any time before 5:00 PM (EST), September 21, 2009.
- Remove the following personal information from your resume and personal essay before you upload them.
- Social security number
- Student ID
- Addresses
- Birth date
- Race, ethnicity, and gender
If you submit an application that is incomplete, the following message with a list of the incomplete sections will be displayed:
Your application is missing required sections. For your application
to be considered, you must complete the sections listed below.
Check your revised application’s content from the View Application
page and ensure your application is complete from the Submit page.
Please note that the Prevention Fellowship Program:
- Does not accept late applications.
- Does not accept paper applications.
- Disqualifies
and/or dismisses applicants and Fellows who are found to provide
misleading or false information on their application.
